Moving on, prepared
Planned & positive transitions into adulthood
A placement should end with a plan, not a date. Transition work begins on day one and is reviewed throughout, so nothing is left to the final fortnight.
Discuss a referralA transition plan from the start
Each young person has a transition plan with goals for housing, income, health, education and support networks, reviewed at every placement review.
Practical housing readiness
We support housing applications, tenancy understanding, utility setup, and where appropriate introductions to leaving care personal advisers and local support services.
Supported endings
Where a young person is ready to move on, we plan a gradual ending with clear arrangements for who they can contact afterwards.
What this includes
- Individual transition plan reviewed throughout placement
- Housing and tenancy readiness support
- Benefits, income and budgeting preparation
- Warm handover to leaving care and community services
Every young person is different
Support hours, staffing and expectations are set around the individual, then reviewed with the placing authority rather than assumed.
